Further, spinal manipulation was backed by three high-quality trials, one systemic review (SR), in addition to two clinical guidelines for acute low back pain. For chronic LBP, four higher quality trials, two SRs, as well as four clinical guidelines found evidence to show pain improvement with spinal manipulation. Additionally, three moderate quality trials corroborated pain improvement with chiropractic means for treating chronic LBP. Among many other findings, this one really stood out though: Of the numerous clinical trials with a variety of invasive treatments, there was insufficient evidence for surgery as a treatment of chronic low back pain. A combined effort for back pain relief works. 241 low back pain patients were randomized to three combos of care: spinal manipulation (SMT) twice weekly for 3 weeks with multifidus activating exercise or spinal mobilizing exercise or more SMT sessions. Significant differences were found for those patients in the SMT plus multifidus activating exercise group. Low back pain and its management is many-faceted.
